(P239) IMPACT OF TRADITIONAL CARDIOVASCULAR RISK FACTORS IN WOMEN WITH OR WITHOUT A HISTORY OF MYOCARDIAL INFARCTION: AN EPIDEMIOLOGICAL ANALYSIS FROM THE QUEBEC CARTAGENE ADMINISTRATIVE DATABASE

Background: While cardiovascular disease (CVD) represented 35% of female premature mortality in 2019, its incidence has been increasing in women. Despite studies assessing sex and gender differences in CVD risk factors (RF), differences in morbidity and mortality persist in contemporary data. At this time, data assessing trends in CVD RF in women with a prior history of myocardial infarction (MI) in the province of Quebec is lacking, yet a better understanding of the RF profile of these women may help reduce observed gaps in morbidity and mortality. The objective of this study was to evaluate the relationship between traditional CVD RF, individually and cumulative, and the occurrence of MI in women.

METHODS AND RESULTS: An epidemiological analysis was conducted using the CARTaGENE database, which collected data from a population-based cohort followed prospectively through questionnaires. 23,793 women were recruited during one of two phases (phase A from 2009 to 2010 and phase B from 2013 to 2014). Analysis was performed to compare trends in women with traditional RF with and without MI. Statistical analyses included chi-square, risk ratio (RR), independent samples T-test, multivariate logistic regression, and descriptive analyses and were performed using SPSS software. Among the 23,793 women (mean age±SD = 53.3±7.9), 197 had a history of MI (0.9%). 85.8% of women with a history of MI were from large urban centers and 56.6% had an annual house income <$50,000. Women with MI were older (58.9±7.4 vs. 53.3±7.9, p< 0.001) and had a higher BMI (30.1±6.9 vs. 27.0±6.3, p< 0.001) than those without MI. Moreover, significant associations were found between MI occurrence and diabetes (RR 4.6 [95%CI: 3.4-6.3]), dyslipidemia (RR 6.3 [95%CI, 4.7-8.5]), and hypertension (RR 7.4 [95%CI: 5.6-9.8]). Finally, women with a history of MI had an average of 2.9 RF for CVD compared to an average of 1.7 RF for those without (p < 0.001).

Conclusion: In a population of women living in Quebec, those with a history of MI were older, had a higher BMI, and a higher number of CVD RF. Timely identification of women with CVD RF may improve prevention of cardiovascular events upstream, with more investigations needed regarding gender-specific data in this population.
