Workshop Description: The cardiovascular continuum follows patients through a progressive sequence of events beginning with risk factors and leading to end stage heart disease. With the evolving demands of patients with chronic and complex conditions, intervention within the sequence of events can be challenging and requires collaborative partnerships. This workshop is presented by the Canadian Cardiovascular Pharmacists Network. This session will lead audience members through common scenarios within a patient’s journey in the healthcare system and highlight opportunities for patient-specific interventions. Where applicable, CCS guidelines will be summarized and applied. Interactive components will include polling, panel discussion and a Q&A period.
Interactive and Engagement Activities Description: This workshop will utilize a real-life patient scenario and integrate polling questions throughout each segment to engage audience members in-person and virtually. There will be approximately 2-4 questions embedded within each section. At the end of the workshop, 15 minutes will be allocated for a question and answer period at the end of the workshop. Audience members will have the opportunity to ask questions, discuss the case with colleagues and share their own experiences with the attendees. If there are no audience questions, the Chair will moderate the panel discussion with questions.
