Workshop Description: Stroke prevention remains a pivotal aspect of cardiovascular medicine, particularly in patients with atrial fibrillation predisposing them to thromboembolic events. Our workshop aims to provide a comprehensive review of three pertinent topics: patent foramen ovale (PFO) closure, catheter ablation for AF, and left atrial appendage closure (LAAC) in high bleed-risk patients.

Topic 1: Left Atrial Appendage Closure for AF in High Bleed Risk Patients: Current Status and Future Directions
Presenter: Jacqueline Saw
LAAC has emerged as an alternative stroke prevention strategy in patients with AF, particularly those at high risk of bleeding complications. This segment of the workshop will discuss the current status and future directions of LAAC in high bleed-risk patients. Topics of discussion will include current indications for LAAC, challenging cases, and real-world case studies highlighting the efficacy and safety of LAAC in diverse patient populations. By examining the evolving landscape of LAAC, we aim to provide attendees with practical insights and guidance for optimizing stroke prevention strategies in high-bleed-risk AF patients.

Topic 2: PFO Closure in Stroke Prevention: Recent Advances and Evidence
Presenter: Anoop Mathew, MBBS
PFO closure has emerged as a therapeutic option for stroke prevention in select patient populations. This segment of the workshop will explore recent advances and evidence regarding PFO closure. Discussions will encompass patient selection criteria, procedural risks, innovation in procedural techniques, and long-term outcomes following PFO closure. Additionally, we will address the management of PFO in patients with underlying hypercoagulable states and in demographics not included in prior studies including older individuals.

Topic 3: Catheter Ablation of AF: Does it impact Stroke Risk and how to Manage Oral Anticoagulation after Successful Ablation
Presenter: William McIntyre, MD

Catheter ablation has revolutionized the management of AF, offering rhythm control and symptom relief. However, the association between AF ablation and stroke risk reduction, as well as the safety of discontinuing oral anticoagulants post-ablation, remains an area of ongoing investigation. This workshop segment will critically evaluate the evidence surrounding the impact of catheter ablation on stroke risk in AF patients and the appropriateness of discontinuing oral anticoagulants after successful ablation. Discussions will include patient selection criteria, procedural outcomes, and long-term management strategies to optimize stroke prevention in this cohort.

Interactive and Engagement Activities Description: Our workshop endeavors to showcase the latest advancements and evidence surrounding stroke prevention strategies, encompassing PFO closure, catheter ablation for AF, and LAAC in high bleed-risk patients. By fostering collaborative discussions and interdisciplinary exchange, we aim to empower clinicians with the knowledge and tools necessary to enhance stroke prevention efforts and improve outcomes for patients with cardiovascular diseases. We invite participants to engage in a stimulating dialogue, contributing to the advancement of stroke prevention strategies in clinical practice.

We will include Q and A sessions after each talk and have audience response system.

Learning Objectives:

At the end of this session participants will be able to:
  • analyze the current indications and future directions of LAA closure in high bleed risk atrial fibrillation patients, enabling the implementation of patient-centered approaches to stroke prevention tailored to diverse clinical scenarios;
  • evaluate patient eligibility criteria for PFO closure, integrating knowledge of recent advances and evidence, to enhance decision-making processes in stroke prevention strategies; and
  • assessing the impact of catheter ablation on stroke risk reduction in AF patients and formulating evidence-based strategies for the management of oral anticoagulation post-ablation to optimize stroke prevention outcomes.
