• HCM I. 54 yr. asymptomatic male marathon runner with HCM Dx age 30 yrs. Stress echo normal 16 METS with rest LVOT gradient 25 mmHg & peak exercise LVOT gradient 110 mmHg a. Any role for BB & calcium channel blocker? b. LGE 20% on CMR? c. Genetic screening routinely for “everyone”
II. 39 yr. male symptomatic hockey player Stress echo significant dyspnea at 16 METS with rest LVOT gradient 25 mmHg & peak exercise LVOT gradient 110 mmHg a. BB or calcium blocker b. Disopyramide c. Myosin inhibitor d. Septal reduction therapy – alcohol ablation vs. surgery
